Waiver and Release of Liability

Note: While Tri-State events are low-risk activities and are intended for parents and their children to participate in together, our insurance company requires that a waiver be signed for participation in the activities. Therefore, this form must be completed before the participant is permitted to take part in any event.
In consideration of my involvement in Tri-State Homeschool, Inc. events, I acknowledge, appreciate and agree that:

1. I am responsible for the behavior and safety of my children.

2. I give permission for my children to attend and participate in Tri-State Homeschool, Inc. events..

3. I and my children risk bodily injury and death as well as risk damage to or loss of property.

4. I knowingly and freely assume all such risks; both known and unknown, even if arising from the negligence of the releases or others.

5. I certify that I as parent/guardian with legal responsibility for my children, do consent and agree not only to my children’s release, but also for myself, and my heirs, assignees and next of kin to release and indemnify the Releases from any and all Liability incident to my minor children’s involvement as stated above, even if arising from the negligence of the releases, to the fullest extent permitted by the laws of the State of Delaware. The laws of the state of Delaware, exclusive to its choice of law rules, shall apply to this Waiver and Release of Liability Agreement
